Archived 4x4s

So just recently, all the gsa's/gstl's were told we need to do 4x4s for certain checklanes every day. A sales floor team lead went through it with me briefly and I got KIND OF the jist of it, but can anyone tell me in detail what should be on that 4x4 sheet and how I need to go about Doing the whole thing properly? Thanks!
