MODERATED AMERICANS WITH DISABILITIES ACT (ADA)

Just out of curiosity, what would you do if the form your hr gives you for accommodations has nothing that you would need for your disability?

The form isn't supposed to be a check box.
It's supposed to be a way for the company and the employee to collaborate on coming up with an accommodation.
They can't say these are the things we'll offer you and that's it.
You put down what you need and if they have a problem with it then they come back with a compromise.
But it has to be a workable compromise.
They don't get to say, no we aren't going to do anything.

I edited this into the main post as well but here is a labor lawyer explaining how HR has to prove that they can't provide accommodations.
