Archived leave of absence

Hey guys...I've got something going on and need to take 3 weeks off. No vacation time to cover that period. How do I go about a leave of absence? Talk to hr? My sgl? Or what? Just getting a heads up from someone who has taken one before or knows thanks

Talk to hr. They'll ask you to fill out a LOA form basically asking why you need off and what dates you'll need off. If its medically related at all, you might look into FMLA instead. Both will be unpaid time off, but if either one is approved then it wont be accountable time. Hope it works out for you bro.

I'll fill out one. Just in your opinion do u think that they will decline my request if it's for cosmetic surgery? And will probably have to save up my vacation hours to do so
