Yeah, all LODs know where the form is seeing as Target has a revolving door.

You could print that out, put it in the paper tray of the copier at the store, and make a copy of the resignation form on the other side.

like everyone says, ask for a resignation form. It might also be a nice touch (but is NOT required) to have a resignation letter to give to HR when you hand in the form.
