So I need to ask for the 13th 14th and 15th of september off. The schedule hasn't been made yet for that week but im confused on who i ask. Can i can Hr and just ask or do i need to ask in person? Im still in my 90 days so does that mean i cant?

Submit a request on mytime on store computer AND talk to YOUR etl.

It gives me a chart after filling a form. It says i must fill up to average hours ti continue. What does that mean?

That’s for paid time off (PTO). If all you need is a day off you’ll want to click the other box for an unpaid vacation request. Then enter the days and either the times you are unavailable or the Full Day/Overnight box. Don’t forget to check the bottom box to acknowledge that time off is pending and business needs always come first. Tell your TL or ETL immediately so they can approve it before it gets auto-denied.
