Archived yearly review format

I tried searching for review format in the search box, went through topics but couldnt find it.
Is there a certain format for yearly reviews (like categories where you are rated from u-o)? Or is it more comment based?

You can get that sample info on workbench having a tl access. Also, a suggestion is to get a copy of your core roles. It will get you, a level of understanding on what spot is looking for. All yearly reviews were done back in April & may. Are you under 90 days? If so, you will get review then. Best bet, be a stellar tm & don't call out.
